$1.5M grant for leading interprofessional collaborative practice for underserved populations

Registered nurses will take the lead in a new program designed to better coordinate chronic care for underrepresented and underserved populations.

U-M School of Nursing, in partnership with Southwest Detroit’s Community Health and Social Services Center, is implementing the new program for registered nurses who are chronic care coordinators. It is designed to advancer patient-centered care by improving communication and documentation among teams of doctors, nurses, pharmacists, support staff and social workers.

“The nurse leaders will work with the teams to develop a sustainable model of interprofessional care that improve chronic care outcomes,”says Donna Marvicsin, clinical associate professor in the U-M School of Nursing, who will serve as the project director.
